Rambo Memorial Health Center Nurses
Are Coming to Tri-Valley High School
Offering TB skin Tests to any HS Senior
FREE OF CHARGE
(some colleges may require & list on their physical forms)
 
Anyone under the age of 18:
Download the consent form HERE
Or pick one up at the High School office
Bring this form WITH PARENT’s SIGNATURE/DOB
With you on April 4th
 
*Skin tests MUST be read on
Monday, April 7th
Rambo Memorial Health Nurses
will return to TVHS between 8:30-9:00am to read these
OR the student may stop at Rambo to have it read on the 7th
